ข้อตกลงและเงื่อนไขการสมัครและการให้ความยินยอมในการจัดเก็บ ใช้ เปิดเผย และจัดส่งข้อมูลส่วนบุคคล

1. ข้าพเจ้ามีคุณสมบัติครบถ้วนตามประกาศรับสมัครของสถาบันเทคโนโลยีนานาชาติสิรินธร ม.ธรรมศาสตร์ ทุกประการ และได้ศึกษากำหนดการอย่างละเอียดก่อนการสมัครเรียบร้อยแล้ว

2. ข้าพเจ้ายินยอมเปิดเผยข้อมูลส่วนบุคคลของข้าพเจ้าที่มีความสมบูรณ์ ถูกต้องและเป็นปัจจุบันทั้งหมดแก่สถาบันเทคโนโลยีนานาชาติสิรินธร ม.ธรรมศาสตร์ โดยข้อมูลส่วนบุคคลของข้าพเจ้าจะถูกนำไปใช้ให้เป็นไปตามวัตถุประสงค์ที่กำหนดคือกระบวนการรับเข้าศึกษาเท่านั้น เพื่อให้เป็นไปตามพระราชบัญญัติคุ้มครองข้อมูลส่วนบุคคล พ.ศ. 2562

3. ข้าพเจ้ายินยอมให้สถาบันเทคโนโลยีนานาชาติสิรินธร ม.ธรรมศาสตร์ ดำเนินการจัดเก็บ ใช้ เปิดเผย และจัดส่งข้อมูลส่วนบุคคลของข้าพเจ้าเพื่อทำการประมวลผลในกระบวนการรับเข้าศึกษาตามความจำเป็น และเป็นไปตามพระราชบัญญัติคุ้มครองข้อมูลส่วนบุคคล พ.ศ. 2562 เพื่อให้ถูกต้องตามวัตถุประสงค์ดังต่อไปนี้

3.1 ดำเนินการจัดเก็บ รวบรวมข้อมูลที่เกี่ยวข้องกับการลงทะเบียนเข้าใช้งานระบบรับสมัคร ข้อมูลที่เกี่ยวข้องกับการสมัครเข้าศึกษา ข้อมูลที่เกี่ยวข้องกับการลงทะเบียนเพื่อออกรหัสนักศึกษา และข้อมูลที่เกี่ยวข้องกับการรายงานตัวเป็นนักศึกษา ทั้งนี้จะดำเนินการจัดเก็บข้อมูลไว้จำนวน 5 ปีการศึกษา เพื่อนำไปใช้ประโยชน์ในการสรุปข้อมูลเป็นสถิติที่เกี่ยวข้องกับการรับเข้าศึกษาเท่านั้น

3.2 ดำเนินการใช้ข้อมูลส่วนบุคคล เพื่อประมวลผลในกระบวนการรับเข้าศึกษา การคำนวณคะแนน การตัดคะแนนตามเกณฑ์รับเข้าศึกษา และการสรุปข้อมูลการสมัครหรือสถิติ เพื่อเป็นประโยชน์ในกระบวนการรับเข้าศึกษา รวมถึงการใช้ข้อมูลส่วนบุคคลเพื่อติดต่อสื่อสาร ส่งการแจ้งข้อมูลทางโทรศัพท์ หรืออีเมล ในกรณีที่เกี่ยวข้องหรือพบปัญหาจากกระบวนการรับเข้าศึกษา

3.3 ดำเนินการเปิดเผยข้อมูลส่วนบุคคลผ่านทางประกาศรายชื่อผู้มีสิทธิ์สอบข้อเขียน/สัมภาษณ์ ประกาศรายชื่อผู้มีสิทธิ์เข้าศึกษา ประกาศรายชื่อผู้ยืนยันสิทธิ์เข้าศึกษา และประกาศรหัสนักศึกษา บนสื่อต่างๆ ของสถาบันเทคโนโลยีนานาชาติสิรินธร ม.ธรรมศาสตร์

3.4 ดำเนินการจัดส่งข้อมูลหรือให้สิทธิ์การเข้าถึงข้อมูลส่วนบุคคล โดยสถาบันเทคโนโลยีนานาชาติสิรินธร ม.ธรรมศาสตร์ มีความจำเป็นต้องจัดส่งข้อมูลหรือให้สิทธิ์การเข้าถึงข้อมูลส่วนบุคคลแก่หน่วยงานภายนอกทั้งในประเทศและต่างประเทศ เพื่อประมวลผลข้อมูลที่เกี่ยวข้องกับกระบวนการรับเข้าศึกษา กระบวนการตรวจสอบวุฒิการศึกษา และกระบวนการขึ้นทะเบียนนักศึกษา อาทิเช่น การจัดส่งข้อมูลผู้สมัครและผู้ผ่านการคัดเลือกไปยังระบบการคัดเลือกกลางบุคคลเข้าศึกษาในสถาบันอุดมศึกษา (TCAS: Thai University Center Admission System) ของที่ประชุมอธิการบดีแห่งประเทศไทย (ทปอ.) เพื่อประมวลผลการรับเช้าศึกษา ยืนยันสิทธิ์เข้าศึกษา หรือตรวจสอบความถูกต้องของข้อมูลผู้ยืนยันสิทธิ์เข้าศึกษา เป็นต้น

คำชี้แจงเกี่ยวกับการให้ความยินยอมในการจัดเก็บ ใช้ เปิดเผย และจัดส่งข้อมูลส่วนบุคคล

1. สถาบันเทคโนโลยีนานาชาติสิรินธร ม.ธรรมศาสตร์จะจัดเก็บ ใช้ เปิดเผยและจัดส่งข้อมูลส่วนบุคคลภายใต้วัตถุประสงค์ที่ได้แจ้งไว้ โดยมีระยะเวลาในการเก็บรวบรวม ใช้ และเปิดเผยข้อมูลส่วนบุคคล จำนวน 5 ปีการศึกษา นับแต่ปีการศึกษา 2565-2569

2. ในกรณีผู้ให้ความยินยอมในการจัดเก็บ ใช้ เปิดเผยและจัดส่งข้อมูลส่วนบุคคลเป็นผู้เยาว์ (อายุไม่ถึง 20 ปี บริบูรณ์ หรือยังไม่บรรลุนิติภาวะตามกฎหมาย) ต้องได้รับความยินยอมจากผู้ใช้อำนาจปกครองที่มีอำนาจกระทำการแทนผู้เยาว์ (ผู้ปกครอง) ด้วย

3. สถาบันเทคโนโลยีนานาชาติสิรินธร ม.ธรรมศาสตร์ ขอสงวนสิทธิ์เปลี่ยนแปลงแก้ไขข้อตกลงและเงื่อนไขการสมัครและการให้ความยินยอมในการจัดเก็บ ใช้ เปิดเผยและจัดส่งข้อมูลส่วนบุคคล โดยจะทำการแจ้งให้ทราบล่วงหน้าอย่างน้อย 14 วัน

4. กรณีที่ท่านมีข้อสอบถามหรือดำเนินการร้องขอเกี่ยวกับการคุ้มครองข้อมูลส่วนบุคคลสามารถติดต่อ ได้ที่

Terms and conditions of application and consent for the collection, use and disclosure of personal information and documents

1. I have all the qualifications pursuant to the Admission Announcement of Sirindhorn International Institute of Technology, Thammasat University, and I have read the schedule carefully before applying.

2. I consent to disclose my accurate, certified, and up-to-date personal information to Sirindhorn International Institute of Technology, Thammasat University, for the specific purpose of the admission process only. This process is to comply with the Personal Data Protection Act, 2562 BE.

3. I consent to let Sirindhorn International Institute of Technology, Thammasat University collect, use, and disclose my personal information for specific purposes, namely, the admission process only. This process is to comply with the Personal Data Protection Act, 2562 BE., which will be for the following purposes:

3.1 Sirindhorn International Institute of Technology, Thammasat University will collect, use, and disclose personal information that is related to the registration for issuing student id, and the information that is related to registration as a student. The period for collection, use, and disclosure of personal information will be limited to 5 academic years. This is to be used for the purpose of summarizing the information as statistical information related to admission only.

3.2 Sirindhorn International Institute of Technology, Thammasat University will use this personal information for evaluation during the admission process, score calculation, making an admission decision, and summarizing an application data or statistics during the admission process, including using personal information to communicate, providing information by phone or email in case of any problems discovered during the admission process.

3.3 Sirindhorn International Institute of Technology, Thammasat University will disclose personal information through the announcement of eligible candidates for examination/interview, the announcement of admitted students, the announcement of confirmed students, and the announcement of student id on various social media channels of the Sirindhorn International Institute of Technology, Thammasat University.

3.4 Sirindhorn International Institute of Technology, Thammasat University will provide personal information or grant access to personal information to external organizations both domestically and internationally. This is to evaluate the related information during the admission process, degree verification process, and registration process. For example, submitting the information of applicants and admitted students to the Thai University Center Admission System (TCAS) and others.

Statement of consent for the collection, use and disclosure of personal information and documents

1. Sirindhorn International Institute of Technology, Thammasat University will collect, use, and disclose personal information in the stated purposes. The period for collection, use, and disclosure of personal information will be limited to 5 academic years starting from the academic year 2022 – 2026.

2. In case that any person who gives consent for the collection, use, and disclosure of personal information is a minor (a person who is the age under 20-year-old or under the legal age), they are required to have the consent of their guardian on behalf of the minor.

3. Sirindhorn International Institute of Technology, Thammasat University has the right to amend the terms and conditions of application and consent for the collection, use and disclosure of personal information and documents which will inform at least 14 days before the amendment.
